Who to refer?
Always offer CPC for these newborns:
- Infant with NAS
- Foster/Kinship Care
- Parent/guardian who is 16 and under
- Parent/guardian with substance use disorder
Consider offer CPC for newborn families with:
- Family currently unhoused
- Active IPV in home
- Maternal ID
- Significant parental mental health/trauma history
For older children who meet this criteria, please message the CPC team on a case by case basis.
